WordPress incident brief worksheet
Make a first incident brief without entering site addresses, personal details or credentials. The worksheet arranges your observations and flags unanswered preparation questions. It does not connect to WordPress, inspect a site or determine the cause.
Answers stay in this browser tab’s memory. Nothing you select is uploaded or saved in browser storage. A refresh clears the worksheet. The worksheet sends no answers or summary text.
Your recovery summary
Choose what you know. Unanswered items will be marked unknown.
Review the text before sharing it. Downloading creates a file on your device. Clear answers removes this tab’s answers and summary; it cannot remove files you have downloaded.
What to add privately after you download
In your own controlled records, add the affected path, expected and observed behavior, timezone, last known good time and first failure time. Keep raw logs and personal records in a protected support channel agreed with the owner. A general inquiry needs only a short, sanitized description.
If a backup or authorization is unknown, that is a question to resolve before repair. An untested backup is not proof that restoration will work. If you suspect active compromise or payment/data exposure, arrange qualified incident support before ordinary troubleshooting.
Choose a test only after preserving the current state
The triage matrix explains the evidence to protect for each symptom. The fictional recovery walkthrough shows why the most recent update is a lead to investigate rather than a diagnosis.
Ask what can be checked first
Use the summary to discuss the affected action and the gaps in your recovery information. See WordPress rescue services or start an inquiry with a short description, without raw logs or credentials.
